본 연구는 족삼리·사관의 지압봉 안압이 학령기 아동의 식체 증상 완화에 미치는 효과를 검정하여, 학교현장에서 활용할 수 있는 한의학적 간호중재를 개발하고자 시도한 비동등성 대조군 전후 유사 실험설계 연구이다. 연구대상은 2003년 4월 1일부터 5월 30일까지 울산광역시에 위치한 D, M, N 초등학교에서 식체 증상으로 보건실을 방문한 아동 중 D 초등학교 아동 35명은 실험군, M, N 초등학교 아동 35명은 대조군으로 하였다.
실험군과 대조군에게 사전조사로 식체 증상을 사전한 후 실험처치로 실험군에게는 지압봉으로 족삼리·사관을 강자극으로 1분간 3회씩(20초에 1회) 안압하였고, 대조군에게는 통상적으로 시행하고 있는 간호중재로 소화제(훼스탈 플러스 1T~800㎎)를 투여하였다. 사후조사로 실험군과 대조군에게 식체 척도를 이용하여 3회(40분, 80분, 120분 후)에 걸쳐 식체 증상을 사정하였다. 본 연구에서 사용한 식체 척도는 대한소화관운동학회에서 채택한 기능성 위장관 장애의 진단기준인 로마기준Ⅱ(최명규, 한석원, 2999; 최명규, 2000)를 한의학적 사정 기준에 적용시킨 후 한의사 2인과 간호학교수 2인의 자문을 받아 식체 증상 척도를 완성하였으며, 연구도구의 신뢰도는 Chronbach´s α=0.87이었다. 식체 척도는 ´얼굴이 황백색이거나 백색이다´, ´먹기를 싫어한다.´, ´배가 포만감이 있고, 더부룩하다´,´명치부위가 아프다´,´속이 미식거린다´의 5가지 증상의 유무로 구성되어 있다. 수집된 자료는 SPP 10 WIN Program을 이용하여 실수와 백분율, x²-test와 independent t-test로 분석하였다.
본 연구의 결과는 실험처치 후 식체 증상이 40분 후에는 실험군은 평균 2.89점이고, 대조군은 평균 3.83점으로 두 군간에 유의한 차이(t=6.317, p=0.000) 를 보였고, 80분 후에는 실험군은 평균 1.97점, 대조군은 평균 3.39점으로 유의한 차이(t=10.653, p=0.000)를 보였으며, 120분 후에는 실험군은 평균 1.26점, 대조군은 평균 2.83점으로 유의한 차이(t=11.648, p=0.000)를 보여 가설이 지지되었다. 이상의 결과로 볼 때, 족삼리·사관의 지압봉 안압이 학령기 아동의 식체 증상 완화에 효과적임이 입증되었다. 본 연구결과에 근거하여 족삼리·사관의 지압봉 안압이 학교현장에서 보건교사가 독자적으로 시행할 수 있는 간호중재법으로 계쏙 활용되기를 바라며 대상자를 확대하여 족삼리·사관의 지압봉 안압이 식체 증상 완롸 효과에 대한 반복연구의 필요성이 있다.
Objectives : The purpose of this study was to identify the relationships between the maternal burden of caretaking and the child-rearing attitude of the infants´ mothers.
Methods : The subjects were 117 mothers of infants who had visited a public health center during the period of July 1st to 30th in 2003. Han´s the Burden of Caretaking Scale and Coiher´s Maternal Attitude Scale were used.
Results :The results of this study were as follows;
The mean score of the maternal burden of caretaking and the child-rearing attitude were 21.7, 63.1 respectively. The maternal burden of caretaking was significantly related with infant birth order (p < 0.05), and the child-rearing attitude was significantly related with infant´s. age (p < 0.10) . There were significant relationships between the caretaking burden and the child-rearing attitude of the infants´ mothers when the effects of maternal education, economic state, number of children, intention to pregnancy and presence of rearing-supporter were controlled(r=-0.318, p < 0.001).
Conclusions : In conclusion, as maternal burden of caretaking was stronger, the subject´s child-rearing attitude was more negative.
